We’ve all seen the warnings; the labels on prescription bottles telling us not to mix the pills with alcohol. The warnings tell us that alcohol may blunt or enhance or nullify the effect of the prescribed drugs.

But what’s so bad about mixing alcohol with common medications? What, really, can go wrong? “Nausea and vomiting, headaches, drowsiness, fainting, or loss of coordination,” according to the National Institute on Alcohol Abuse and Alcoholism (NIAAA). “It can also put you at risk for internal bleeding, heart problems, and difficulties breathing. In addition to these dangers, alcohol can make a medication less effective or even useless, or it may make the medication harmful or toxic to your body.”

The NIAAA reminds consumers that certain medicines, such as cough syrup and laxatives, may contain up to 10 per cent alcohol to begin with. Moreover, older people are at particular risk, since the body breaks down alcohol more slowly with age. Woman are also at high risk for drug/alcohol interactions, since blood–alcohol levels are typically higher in women than in men after consuming the same amount of alcohol.

Finally, the NIAAA advises, “Timing is important. Alcohol and medicines can interact harmfully even if they are not taken at the same time.”

Herewith, a partial listing of drug/alcohol reactions to watch out for, taken from the NIAAA’s publication, “Harmful Interactions: Mixing Alcohol with Medicines.”

Possible reactions with alcohol:

–Angina: Isordil, Nitroglycerin. Rapid heartbeat, dizziness, fainting.

–Anxiety and Epilepsy: Librium, Valium, Xanax, etc. Increased risk of overdose, difficulty breathing, impaired motor control, memory problems.

–Blood clots: Coumadin (Warfarin). Internal bleeding, strokes, heart attacks.

–Depression: Prozac, Zoloft, Lexapro, Wellbutrin, etc. Drowsiness, dizziness, increased risk of overdose.

–Diabetes: Glucophage, Orinase, etc. Abnormally low blood sugar, flushing reaction, nausea, vomiting.

–Arthritis: Celebrex, Naprosyn, Voltaren. Ulcers, stomach bleeding, liver problems.

–High blood pressure: Catapres, Cardura, Lopressor, etc. Dizziness, fainting, arrhythmia.

–High cholesterol: Crestor, Lipitor, Pravachol, etc. Liver damage.

–Infections: Acrodantin, Flagyl, Grisactin, Nizoral, Nydrazid, Seromycin, Tindamax: Rapid heartbeat, stomach pain, vomiting, flushing.

–Severe pain: Demerol, Percocet, Vicodin, etc. Drowsiness, dizziness, increased risk of overdose, difficulty breathing, impaired motor control.

–Sleep problems: Ambien, Lunesta, Sominex, etc. Dizziness, difficulty breathing, impaired motor control, memory problems.

–Enlarged prostate: Cardura, Flomax, etc. Dizziness, fainting.

And don’t forget the herbals: Alcohol with Kava Kava may cause liver damage; alcohol with St. John’s Wort risks dizziness and overdose. Chamomile, valerian and lavender will increase drowsiness when mixed with alcohol.

It is very essential for everybody to be aware of a detailed account of the attack of major (or grand mal or generalized), focal or Jacksonian and petit mal epilepsies. At times, tests like electroencephalogram (EEG) and / or computed tomographic (CT) scanning may be normal in a case of epilepsy, and, therefore, it is only the evidence of the eyewitness, who is in a position to report the details to the attending physician, when the patient is brol1ght to the hospital. As an emergency case, a clinical diagnosis of epilepsy can be made on the spot. Moreover, such cases/emergencies need immediate treatment, and the tests, even if they are conducted, take their own time.

In major (or grand mal or generalized) epilepsy, after momentarily experiencing a peculiar sensation or feeling, i.e. one of the types of auras, described earlier (the aura may even be absent), the patient loses consciousness instantaneously, and he collapses all at once. And if unluckily, the place is odd, e.g., a patient is coming down the stairs or standing near a fire or on a hill top, he may injure himself badly.

Following the fall, all the muscles of the body, i.e. all the muscles of the limbs, abdomen, chest and even face become rigid/ stiff. Due to the spasm of the trunk/respiratory muscles, breathing almost stops, and as the muscles of the face are also involved, the mouth gets firmly closed, and the teeth get clenched. The patient then experiences acute agony, and is close to death. But within a few seconds, all the muscles relax, breathing is regained, and the patient starts getting convulsions of the whole body, which usually continue for about 1-2 minutes, and thereafter, the whole body gets relaxed or becomes absolutely limp.

During such convulsions, the tongue moves repeatedly inside the mouth, and together with the repeated movements of the jaws, the saliva in the mouth gets stirred, and frothing occurs. The tongue may also be bitten when it comes in-between the teeth, and sometimes bleeding may also occur, causing a blood-stained froth. The patient may pass urine (and rarely faces) during this phase of convulsions. It should be remembered that tongue-bite, frothing, passing of urine are important clinical features of an attack of epilepsy, which must be noticed carefully by the eyewitness, for the information of doctor.

The patient may sleep for hours following an attack. However, he may be awakened after a reasonable time when he may complain of a marked headache and pain in all the muscles. When awake, he may have no recollection of his severe fall or convulsions. At the most, he may remember the aura, i.e. transitory symptoms that he may have experienced before the onset of the attack of epilepsy. However, this information about the aura has a great significance in clinical diagnosis, and, therefore, the patient must be coaxed again and again to elicit the required information.

The patient may even get the attack during sleep, and in such cases, tongue-bite or passing of urine in bed should help in the diagnosis. The patient may even die due to suffocation if he happens to turn over during this nocturnal attack of epilepsy, with his face buried in the pillow.

Attacks of grand mal epilepsy may occur many times a day. However, the occurrence of attacks of epilepsy are quite variable to the extent that the patient may not have any further attack during his/her whole life. And hence the value of mass awareness about the clinical diagnosis of epilepsy increases because the treatment cannot be started when a single/first attack of epilepsy occurs, which remains doubtful due to the lack of evidence. On the other hand, the patient may get one attack after the other without regaining consciousness in between the attacks, called status epilepticus, which could be even fatal, if immediate medical aid is not provided.

At times, there is a transitory jerky movement of the limb / s, so that in case both the lower limbs are involved and the patient is standing, he may fall down all at once. This state is called myoclonic epilepsy. Sometimes the small muscles of face, eyes etc. may also be involved.

In petit mal epilepsy, which occurs often in children, there is an extremely transitory attack of unconsciousness. The child, if standing, does not even fall down, and if he/she is doing his/her home work, the child continues with the work as if he/she has had no interruption. The attack of epilepsy may occur repeatedly even 15, 20, 30 times a day. When the attacks are limited to 1 to 2 per day, or on alternate days or even less, the child may remain unaware of such an attack. And even if he/she brings it to the notice of the parents, it may be ignored altogether.

However, whenever a child brings the attack to the notice of his parents, they. must take it very seriously, and tests like EEG, CT scan etc. should not be delayed. Therefore, the knowledge of symptoms of petit mal epilepsy is most important for parents, and even for teachers in schools, so that the disease may not remain undetected especially in early and occasional cases. However, in rare cases, the child may even collapse during unconsciousness. This is called an akinetic attack/ epilepsy, and thus, in such cases, there is usually no difficulty in diagnosis.

The attacks of petit mal epilepsy may disappear altogether on attaining maturity, although the treatment will have to be commenced as soon as the diagnosis is confirmed.

It is urgent that all persons, particularly all members/ friends/colleagues concerned with the case must keep in mind all the relevant signs and symptoms whenever the patient is to report to the physician for diagnosis.

Expired medications are commonplace and inertia notwithstanding, many of us tend to rely on an intuitive sense of their value in continuing to store and use them. Such drugs can be harmful to health in several ways; they can be unpredictable in effectiveness, simply ineffective, or even toxic.

 

The formal way of classifying a medication as having expired is through it’s labeled expiry date. This date is often set based on a combination of the common properties of the dosage form as well as the stability and expiration studies of the product that have been conducted by the manufacturer. Importantly, this expiry date is contingent on specific storage conditions of the product. Although a medication may pass it’s labeled expiry date, it may not necessarily be any less effective or dangerous to consume depending on the product itself, the storage conditions and the circumstances leading up to expiry.

 

When most medications pass their expiry date under appropriate storage conditions, they are generally taken to have become so variable in effectiveness as to have become unsuitable for use. This often comes about as a result of the degradation of the active ingredients of the medication with exposure to physical, chemical or microbiological variables like temperature, pressure, humidity, light, bacteria as well as other components of the product known as excipients.

 

Creams may “crack” once their expiry date is passed, leading to a separation of the components and hence provide a non-uniform delivery of active ingredients. This can lead to the poor control of conditions like eczema or acne. While the expiry date provides a useful gauge of when to stop using a medication, there are also many other factors that can informally accelerate the expiry of a medication and make it dangerous to use, chief among which is how the medication is stored. It is oftentimes not just the medication that is affected by storage conditions but also the storage container. Under inappropriate storage conditions, certain containers can leech material into liquid medication preparations, or medication particles can stick to the container rather than remain separated. On average, a 10 degree rise in temperature doubles the rate of chemical reactions that occur to a medication product and can accelerate the rate of bacterial contamination several fold. Just like an ice cream can simply melt or a loaf of bread becomes mouldy much quicker if not refrigerated, many medication products can easily expire much faster when not stored appropriately.

 

With oral liquid and topical medications, potentially dangerous changes associated with expiry can at times be detected by color or consistency changes, component separations, altered smell or taste (oral preparations). Should a suspicion of expiry arise, a medication expert should be consulted regardless of whether or not the labeled expiry date has been passed.

 

“Expiry” should also be understood to occur once a supply of medications is no longer used appropriately for it’s intended purpose. Consultation with a medication expert is always advised to prevent the inappropriate use of existing medication supplies. Inappropriate use can often occur with self-medication and is harmful. An unfinished supply of a previously used antibiotic may be tried to treat a new infection that is actually untreatable by or resistant to that antibiotic. This practice may not only delay recovery but can also encourage the proliferation of “super bugs” that have resistance to many antibiotics. Another incorrect purpose involves sharing medications and this can be especially harmful if another is allergic to the shared medication or a child or pet is medicated with an adult’s medication. Children often require dose adjustments to accommodate their size while many human drugs are often unsuitable for pets. Even a simple food like chocolate that we may enjoy can easily be toxic to a pet dog.

 

Another mechanism whereby medication expiry is dangerous occurs when an unfinished supply is used despite new information that points to increased precautions associated with the medication or that has led to it’s recall. An example is obtaining pain relief from a previous supply of a painkiller like Vioxx (rofecoxib) or Celebrex (celecoxib) in spite of an existing heart condition that is now known to relate to an increased risk of fatality under those circumstances of consumption.

 

Expired medications that are kept instead of discarded not only take up space but can actually discourage the appropriate use of new supplies in the treatment of illness. A medication cabinet, if not tended to regularly, could eventually contain more expired medications than viable ones and this can lead to the accidental consumption of an expired medication in place of a viable one. It is definitely advisable to clear the medication cabinet of expired medicaitons at least annually if not more often.

 

A further danger however, lies in how expired medications are disposed of. Expired medications and pharmaceutical byproducts can be harmful to the environment especially when they end up in our rivers and drinking water supply. Hormonal compounds like estrogen from birth control pills and patches as well as antibiotics have been linked to being flushed by individuals and institutions into sewage, draining largely unchanged and collecting in rivers and streams, then returning in tiny amounts into drinking water. Traces of antibiotics could worsen bacterial resistance while estrogens and other steroids are known to change the reproductive characteristics of fish. Even trace amounts of chemotherapy medications have emerged in tap water and this could be severely detrimental to the unborn babies of pregnant women who drink such water. The long-term impact on human health of medications in our rivers and drinking water is as yet unknown but no one would want to wait to find out. We can all play our part by inquiring on and using pharmacy or state-run programs for the disposal of expired medications instead of sending them down the sink or the toilet bowl.

 

A pharmacist is the expert of choice to approach in handling medication expiry and should be consulted if in doubt. As a general rule, it is always best to safeguard your own health and that of those around you by expeditiously and appropriately discarding all expired medications.
